Guest Player Form (NOTE: Both coaches must sign form. The player’s card, and a NEW medical release must be presented to the NTSC registrar for release to guest play. FOR NTSC PLAYERS, a NTSC REGISTRAR MUST RED STAMP THE FORM RELEASING THE PLAYER TO GUEST PLAY.)
